Apply for Review by the Commissioner (off-line)

You may choose to apply to the Information Commissioner for review of a decision made by a public body by downloading the form underneath.  The completed form should be addressed to:

Office of the Information Commissioner, 18 Lower Leeson Street, Dublin 2 

You must enclose the relevant fee if one is payable. If you do not enclose the fee we cannot accept your application. Please pay by cheque or postal order. Do not enclose cash.
